The MDS Manager is responsible for Health Center assessments in the transitional care unit. Complies with La Posada Code of Conduct in all aspects of job including participation in compliance training as offered and understands responsibility to report suspected violations through the chain of command, or Kisco Compliance Hotline, or Written Report of Suspected Compliance Violation, or contacting the Corporate Compliance Officer directly.

What will I do every day?

• Follow-up all Federal mandated Medicare rules and regulations for admission and discharge eligibility.
• Coordinate chart for residents, assuring care plans are completed. (Interim care plans and comprehensive care plans)
• Assumes the authority, responsibility, and accountability of organizing and directing MDS/Care Plan Activitie
• Complete MDS for all residents.
• Prepare all Medicare requirements and PPS documents.
• Provide Training to nursing as needed on areas of MDS and careplan.
• Participate in various patient care committees as assigned. · Conduct careplan meetings with families and staff.
• Care plans are appropriate to resident needs and complete with measurable goals and individualized interventions. ·
• Ensure that all MDS's are properly locked within required time frames.
• Participate in all meetings, in-service seminars and committees as designated by management.
• Transmits MDS's, holds weekly Medicare meetings.
• Electronically transmit the MDS in compliance with IDPH and Quality Indicator requirements.
• Care and manage insurance patients.
• Collaborates with therapy staff to maximize patients’ rehab potential.
• As well as perform other related duties as required

What will I need to be successful in this role?

• Must of a RN License
• Have a minimum of three-years experience in Short Term Skilled Nursing environment.

Special Requirements/Certifications I may need?

• Current active, unencumbered RN license in the state of Florida
